Medical statement
‍A medical certificate is a written document that states whether or not someone can perform certain actions. As your doctor, we are not allowed to issue a medical certificate for our own patients. For more detailed information, please refer to the website of the KNMG. Here you will also find the so-called refusal note that you can hand over to third parties, such as your employer or school.

‍Request for information
‍Does a medical advisor or lawyer need medical information for an assessment? Then a request can be submitted to us in writing. It is important to know what information is being requested, which is usually stated in the consent letter. We never provide information to third parties without your permission, including family members. We will process this request within 4 to 6 weeks.

‍Do you need an explanation for medical urgency? Then we refer you to the Rijnmond Housing Seekers Urgency Foundation. In the Rotterdam region, this foundation is responsible for granting medical urgency to home seekers.
