Consent to share data

At Health Center Sportdorp, we process personal data in order to treat you properly and to arrange financial processing. Sometimes this is also necessary to prevent a serious health hazard or to comply with legal obligations.

According to the GDPR, we are responsible for the careful handling of your data. This means that we:
- collect data only for clear purposes, such as care and policy,
- do not use your data for other purposes without your consent,
- inform you about how we handle your data,
- keep your data strictly confidential,
- communicate with you directly as much as possible. Only if you are medically unable to speak to us yourself can you give permission to someone else (e.g. a partner or child).
- secure your data properly and do not store it longer than necessary.

You have various rights under the law. You may:
- know what data we process,
- view your data and receive a copy,
- have your data amended or (partially) deleted,
- ask for (partial) destruction of your medical file,
- add a personal statement to your file,
- object to the use of your data in certain cases.

We only share your information confidentially, and only with your consent or when required by law. Sometimes data is shared with other healthcare providers, for example via the National Exchange Point (LSP), to ensure your medication safety. This only happens if you have given permission to do so.
