House rules
You are more than welcome to visit our health center. Our starting point is that we treat everyone who contacts our health center with respect, but we also expect the same from our patients. By entering our health center or contacting us, you agree to the rules below. This way, we make it as pleasant as possible for everyone and we can do our best to provide you with the best possible care.
House rules
1. Be respectful — no aggression Treat others with kindness and respect. Scolding, discrimination, unwanted touch or bullying are not allowed. Threatening, intimidation, or aggressive behavior is not accepted. In case of aggression or violence, we take measures. We report serious cases to the police.
2. Safety at the health center Always follow the instructions of our employees, especially in the event of an emergency. Smoking is prohibited in and around the general practice. Alcohol and drugs are not allowed. Are you under the influence? Then we can refuse you. Weapons or dangerous items are prohibited.
3. Privacy and phone use
Do not take photos, videos, or audio recordings without permission from our staff or other patients. Keep your distance and respect the privacy of others. Set your phone to silent in the waiting area. Don't use sound. Do not use your phone while talking to the doctor or assistant.
4. Fulfill the appointment
Are you unable to come to your appointment? Please cancel at least one business day in advance. Didn't cancel on time? Then we charge costs:
€15 for a regular consultation
€30 for a double consultation
€45 for a special consultation (for example a procedure)
In case of force majeure, we do not charge anything. Please let us know as soon as possible.
5. Identity and minors
Everyone over the age of 14 must be able to show a valid ID. Children under 16 can only be registered if one of the parents or guardians is also registered with us. After a divorce with joint custody, both parents must give permission to register or deregister. Young people from the age of 16 can make their own decisions about their care and sign forms.
6. Property and damage
Pay close attention to your belongings. We are not responsible if you lose something or if your items get damaged. Handle our furniture, appliances and other items in the health center properly. Are you causing damage to our health center or borrowed items, intentionally or through careless behavior? Then we can recover the costs from you. Even if we only discover this later.
7. Pets
Pets are not allowed in our health center. Only service dogs are welcome.
Consequences
Are you not following the house rules? Then we will talk to you about your behavior and we can give you an official warning. We can also ban you from accessing the health center. Are you still not complying with the house rules after that? Then you will receive a final official warning. If that does not work and you systematically do not comply with our house rules, we will unilaterally terminate the treatment relationship as described in the national KNMG guideline (in other words, we will unsubscribe). You are then no longer our patient and we will no longer provide care.
Please note:
We do not accept any form of physical violence or life threats, in which case we will unsubscribe you immediately and report it to the police.
