Do you want to unsubscribe from our practice? Please follow the steps below to properly arrange your unsubscription.
Step 1: Find a new doctor
Register with a new doctor in your place of residence.
Step 2: Let us know you're leaving
Then let us know that you want to unsubscribe from Health Center Sportdorp.
This can be done via: your online file at myhealth.net, or by phone via the doctor's assistant.
Step 3: Permission to transfer a file
Your new doctor will request your medical file from us. We are only allowed to send this if we have received permission from you. Unfortunately, we cannot transfer your file without permission.
The transfer of the file is digital and secure. Not a new doctor? If you unsubscribe without a new doctor, we will not be able to forward your file. By law, we are obliged to keep your medical data for 15 years.
